Parking – For your convenience, a parking lot is located at the front entrance of our building. It is handicapped accessible.
Cancellation Policy
If you are unable to keep an appointment, we ask that you notify us, preferably at least 24 hours in advance.
What do I do when I arrive for my appointment?
When you arrive, check in with the receptionist. Please notify if there are any changes in your name, address, telephone number or insurance information. You may also update this information on your patient portal.
If I am a new patient, how do I prepare for my appointment?
New patients have an intake form that will need to be filled out. Please access it here. We utilize the MEDENT portal which will allow you to update your information electronically, right into the medical record If unable to access the portal, please drop off or mail the form prior to your visit to improve the efficiency of your visit.
How do I get prescriptions and refills?
Please bring all of your prescription bottles to all appointments. Call your pharmacy for any refills and they will electronically request for you.
How do I get my test results?
All patients should be notified of abnormal test results via a phone call or letter if unable to get in touch. If you haven’t received a call in a timely fashion, you may call the office for an update.
Common studies via imaging, labs are usually returned within a few days. Other less commonly ordered tests have to be sent out to another lab facility and may take up to a few weeks.
What do I do in the event of an emergency?
For serious emergencies, call 911 for ambulance service / EMS evaluation. They make need to take you to the nearest emergency room for evaluation by an ER physician or staff. ER services are available 24 hours a day, 7 days a week 365 days a year.
Can I call the office for a problem?
We will answer any questions by phone that we can. Physicians are discouraged from giving medical advice on any new problems without first examining you and may require an acute visit.
How do I pay for services?
Payment of services are due at the time of visit. Insurance contract requirements including collecting all co-pays at the time of service.
What insurances are covered?
Most commercial insurances will be accepted including Highmark BC/BS, UPMC, Geisinger. The list will updated as credentialing continues.
